Essential Functions


Trains, supervises and evaluates Direct Support Professionals and maintains appropriate staffing levels.

Obtains and maintains certification as a Delegated Medication Passer, CPR and First Aid

Reviews all documentation.

Assists in the development of individual service plans (ISP) and attends ISP meetings.

Ensures that safety protocols for the home are in place and adhered to.

Monitors and assures cleanliness and safety of the home.

Facilitates banking activities of clients.

Monitors and facilitates menu planning and shopping based on nutritional guidelines.

Completes all record keeping requirements in compliance with all County Board, ODMR/DD licensure standards, ODHS/Medicaid requirements and OLW requirements.

Carries agency phone (subject to 24 hour on call status) and responds within agency timeframe guidelines.


Support Duties:

Advocates on behalf of the consumer; upholds respectful and efficient communication with consumers and all relevant parties.

Provides emotional support by developing an age-appropriate relationship with consumers.

Assists consumers in maintaining a clean and pleasant home by performing basic housekeeping and deep cleaning duties.

Aids consumers in developing and maintaining daily living skills encouraging independence, hygiene (i.e. bathing, toileting, dental care), appearance (i.e. care of clothing, laundry, appropriate dress, etc.), behavior (i.e. appropriate responses/actions with others including other consumers and the public), proper diet/eating habits, the preparation of meals, and safety (fire drills, first aid techniques).


Performs direct support and supervision, including but not limited to:

Providing transportation for medical, dental, social activities and other appointments.

Adhering to consumer's rights, mandated reporting and unusual incident requirements.

Providing supervision during community and recreational outings.

Attending consumer related meetings as needed.

Observing any changes in consumer status/well-being and report immediately to Direct Support Manager and/or other individuals as appropriate.

Minimum Qualifications


A High School Diploma or equivalent is required.

A Bachelor’s Degree or equivalent experience involving employee supervision required.

Experience within a program for the MR/DD population is a plus.

Basic knowledge of human development and behavioral management.

Demonstrated leadership qualities and good judgment.

Communicate and interact with consumers, families and staff maintaining the highest level of professionalism, respect and service.

Ability to deal with pressure, problems, and difficult situations, while maintaining perspective and emotional stability.

Basic knowledge of computer usage and applications.

A valid Ohio Driver License with no more than 4 points.

Two (2) years of licensed driving experience may be necessary.

Clear criminal background and drug screening.

Must pass physical screening.
